While on his way to school to confess his love to classmate Kusanagi Mikuru with a love letter, a second year high school student Takatou Iori gets run over by a truck to save a cute first year elementary schoolgirl.
But when he wakes up, he's revived by a perverted doctor, as a full fledged female first year student and is now in the same class as the girl he saved.

It's certainly true this manga hasn't been done before.
Plot rundown (spoils first chapter):
Spoiler (highlight to view)
Studious high school boy who has a crush on a girl in his class nearly dies saving the life of a cute elementary school girl, gets his body reconstructed into that of an elementary school girl and put in the same class as the girl he saved, and ends up living in the same house as the lolicon female doctor who revived him along with her sister who turns out to be his crush.
Ichinensei ni Nacchattara is definitely lolicon (as in, a lot of the content is gratuitous fan service produced to appeal to lolicons) but it's infinitely more tame than Kodomo no Jikan. As far as plot goes... It's not Pretty Face although I can understand the similarity. Rather, it's just a bunch of weird and entertaining situations involving Iori's new life. No real risk of discovery since Iori's body is fully female, and I can't see any serious consequence if there were. And the elementary schooler Rin and Iori and Iori's little sister (note the omission of his crush, Mikuru) are the only characters who are developed at all, really, in the first volume. I read this for genderbender, in which respect it resembles My Barbaric Girlfriend quite a bit (in the "poor abused protagonist" department, mainly, although Iori is mostly different Akira).
Kiddie Pretty Face.
Quite funny, and the lil kids are so cute and innocent but that's expected . Sunfizz98 hit the nail on the head, it's Pretty Face but just in a younger set up.
I only hope that we won't have an open ending like Pretty Face, but the pairing here has even a bigger hurdle to jump then the pairing in Pretty Face seeing the agegap. Tho Rin is cute as hell with her pure eyes.
Lost for Words...
I love the fact that the setup is a bit like Pretty Face, but wow....I have a feeling that the protagonist's mind is going be messed up after a few volumes in.
O shi-!!! Spoilers

A wonderful take on gender bending comedy!
This series shows great promise in its perverted humor. The art is unique/takes a bit of adjusting to but its expressiveness works with the story, such as it is. If the series goes on in this direction it'll keep the high score I give it. Messed up humor for messed up people and I'm one of them.
