Damn this is harder than I thought since I gave my highest rating to at least 11 shounen-romance manga's 
Since I don't like to choose among them, I'll just give you my top 5 not-so-well-known-as-the-others-shounen-romance manga.
Here are those 11 manga's, the top 5 (in no particular order) is marked and in bold:

I actually don't like shounen romances much... But I love seinen ones. May I offer my top 5 seinen romances, instead?
1. Eternal Sabbath: Brilliant female scientist and a psychopathic experiment gone wrong. A romance for the ages!
2. D-ASH: A young pervert and his childhood crush. Sounds like most of us, eh?
3. Densha Otoko: The ultimate otaku finds love with a pretty woman, following an unexpected act of heroism.
4. Emma: A young maid wins the heart of an aristocrat. Almost too gentle for my tastes, but the moe factor beats all. <3
5. Basilisk: Martial arts, star-crossed lovers and mutually assured destruction... A true Armageddon-style romance.
I hope you didn't mind the fact that I listed seinen titles instead. Hmm...

Then add that to my choices. I really thought it might be josei or something ...
Umm others ... I'd also have to go with Train man/Densha Otoko. That one is really sweet.
And Ah! My Goddess, though its not listed as seinen here.
Shounen Romance ...

My top 5 is going to be pretty boring... all Adachi manags. I'll try to at least rate them in order =P
1. Rough
2. Katsu
3. H2
4. Cross Game
5. Hiatari Ryoukou
Well, the last one was a little tough... I almost put Pretty Face ahead of Hiatari Ryoukou.

5. Rave (ok so its not really all romance but there is within the story)
someone mentioned ah megami-sama and that would be my #1 but i dont really consider it as shounen, not really perverted or anything, more like shoujo thats borderline shounen
