For most of the story, and yet, they do go to Japan briefly. ^_^ Just briefly... But wait, I think they'll probably have to go back to Nihon again just for the fact that...
Spoiler (highlight to view)
DGM Japan is the Earl's country. If majority of the country is full of Akuma, then... It's a fight for humans to take back Japan from Akuma.
A manhua that takes place in Greece: The Prince's Cactus
I haven't yet read anything that took place entirely in the U.S. There are so many that don't take place in Japan, but in some technical "other dimension" or "made up country"... Plus, even if not taking place in Japan, some characters still have Japanese names.
Seinen mecha: The Big O (Although, I've only seen the anime version, which is the original, so not sure how this adaptation goes.)
Tbe shoujo/josei fantasy examples:
The Scarlet Chair
The Empty Empire
Akagami no Shirayukihime
Tokage Ouji (Lizzard Prince)
Pandora Hearts
07-Ghost
Since I lack providing US setting mangas, here are some that does feature bits of U.S., but doesn't take place in the U.S.
Shin Tennis no Oujisama (Prince of Tennis sequel) - really wish the person, who helped the mangaka work on the English bubbles, did a bit more of the accuracy of the NYC subway...
Skip Beat! - interior of a house and an unknown street most likely taking place somewhere In Los Angeles. -.-; Wish the mangaka the best if the story makes the characters go to Hollywood.
B-Wanted - Las Vegas. Didn't expect it, but where else would you see a Las Vegas screentone?

In Eyeshield 21 they go to America to train for a bit. It mostly involves running along a flat, empty desert in Nevada, since they end up in Las Vegas XD

Hetalia Axis Powers: it's set everywhere, because it deals with anthromorphic countries of the world Hilarious history centered around WWII, with Italy as the protagonist (Hetalia apparently means "stupid Italy").

100% Perfect Girl and First Lady.
Both are in other countries (I've forgotten where).

- The One (mainly set in China, but has a bit of Japan in it)
- Candy Candy (a manga set in America)
- Goong (set in Korea)
- The Prince's Cactus (Greece)
- Ingenuo (a bit everywhere, about photograhy, but it's based in Japan)
- Marmalade Boy (At first I thought that it was set in America, because of the
clothing, the way it was presented, but then I found out it was Japan) -_-
Marmalade Boy and Skip Beat! Are mangas that are set in Japan but have a completely foreign feel to it.

Cesare-historical Renaissance Italy
1/2 Prince, mostly online, RL set in China
Red River - around ancient Mesopotamia(?)
Shokoku no Altair...middle east...-ish...
Trinity Blood-futuristic Europe
Forget-me-Not - I would guess Venice, just from the gondolas
Manga set in the Middle East (strangely large amount of yaoi thought...)

MÄR Starts off in Japan, though the majority of it takes place in another world. It's a really awesome series.
