I am looking for manga that treats abuse in a serious, adult way. So that may be josei, seinen, yaoi, yuri, or whatever - as long as it doesn't have a shoujo/shounen like approach. Reading about the character being hit, and how they feel about it.
When I have to give examples I'd say;
Ano hi no Kimi wo Dakishimeta Nara > The destructive way Hidetoshi always ends up with the wrong boyfriends, getting beaten.
Arigatou > The dysunctional family the father comes home too. The way his daughters are treated.
Cut > Dysfunctional teenagers
Endo Hiroki's Tanpenshuu > For Those of Us Who Don't Believe in God > about how the drama group is confronted with their own life.
Kizu > The adult way a subject like child abuse is approached.
Mars > The way Kira is treated by her step father. The way Rei's mother behaved towards kids.
Wild Adapter > vol 5: The abusive relationship between Shota and his mother.

Zankoku na Kami ga Shihaisuru - Definitely
Bokurano , Loveless - But in this case, the details about abuse and suchlike are used as an aid to the main story, not as a primary plot focus. Still, there is a good bit of it; and very realistically portrayed, too.

I second Loveless. As Calstine said, it's not the main focus of the manga, but it does get screen time and there are several examples of children being abused, metally and physically, by the adults around them. It's rather well done.
Asian Beat - This is one volume but chapters 2 onwards deals with abused teenagers.
Banana Fish - Definitely good for realism.
Let Dai - Physically the abuse in this mainly stems from the teenage gangs, but this is very emotionally driven. It's very coming-of-age against parental/societal burdens, that's the best way i can describe it really since I don't want to give anything away but check it out.
17 - maybe since it has a shoujo theme to it but it's only just getting to the serious stuff.
Houkago Hokenshitsu - Well this might interest you. I'm loathed to explain why since no doubt I will spoil it entirely but it is rather good so I suggest you just try reading it.
Shounen zanzou - Boys Next Door- Lawrence is physically abused by his brother.
Fruits Basket - Well if you're talking being psycologically screwed with then Akito's treatment of, well pretty much everyone, but especially Kyo and Yuki, is well potrayed. I'm hesistant in suggesting this, however, because the main female is irritatingly cheery and forgiving. It just makes the whole thing seem ok and detracts from any realism. So I would suggest this if you've grown depressed from reading more tormenting manga but still want to stick to the theme of abuse/angst.

Ash was sexual abused from a young aged and forced into child prostitution.

Parental/Domestic Abuse:
Miyori no Mori - This manga handles parental neglect (which i think is a form of abuse). A ten year old girl is abandoned at her grandmothers.
Second A Cruel God Reigns - Jeremy is sexually abused by her mother's boyfriend for a long period of time.
Naked - Coming of age story about three friends, one of which is abused by his father. I think this is handled pretty well.
Piano no Mori - Piano prodigy kid lives with his prostitute mom and is harassed by her customers. Not central topic. Maybe a bit unrealistic.
Kimi ga Uso o Tsuita - Girl's friend is abused by her parents.
Hoshi wa Utau - There was verbal abuse i think.
Kare no Ondo - Abused by father
