I've seen/read nearly all the anime/manga listed here, but if you truly (I MEAN REALLY) want to rage then watch Tokyo Majin Gakuen Kenpuchō Tō. I've watched it to the end and I walked away cursing. I mean SHOUTING cursing. That's how bad they messed up that anime.

I mean WTH? thats like the epitome of rushed ending- I mean yeah, I put up with you after Sai left and I get THAT!! And the last panel soo ambigious... Still a great manga though

Manga Mitaina Koi Shitai!... I just... ugh. The whole thing.
Oh, and Asa mo, Hiru mo, Yoru mo.
Of the others mentioned... well, I wasn't particularly fond of Hana to Akuma, but I didn't think it was that bad. Hot Gimmick's main character and end pairing made me majorly rage. And as for Code Geass, I cried a ton but I was pretty pleased with the ending.

Naruto just because it was genuinely shit. I have to be honest I only watched 5 episodes but if that is not enough to get me into the series then it just isn't for me. And in Death note killing off L and replacing him with a blonde faggot called mellow and an L wannabe with white hair called Near I mean WTF!!!

The first time this happened to me was with Berserk, Spoiler (highlight to view) when casca was violated by Griffith and Guts was watching and his arm was stuck and there was nothing he could do, until he cut his own arm, I wanted to KILL that bastard myself

The first huh. I'd say when Viz dropped Patlabor after I bought two of their badly done volumes for manga. The second was when my Patlabor movie one tape got stuck and shredded in my VCR. The VHS left to right days of manga/anime imports were very bad indeed.
As for actual events in manga/anime, I really can't say I've ever raged. I've been disgusted like when I find the surprise addition of yaoi in a hentai I was enjoying but that is just about it.
Edit: Scratch that. I recall watching Peach Girl pissed me off.

Bloody Monday 2. The first one was awesome! I loved it! and then... it all went downhill.
Spoiler (highlight to view) especially when the main guy fell in love with that body guard girl.... it was so uncalled for, and totally rushed, and made no sense whatso ever. I raged, and I stopped reading it.
And, also, Hunter x Hunter. One of my favorite mangas! But I rage at the author for going on a bajillion hiatuses. :'|
