What is the best way to translate a dialect or accent of the original language that is not the norm (e.g., Kansai-ben)?

I'm looking for this short story where a student bangs up his fist because he punched a wall. The school doctor takes care of it and I forgot the rest but I remember it turns out the the student loves the school doctor and in the end, they have sex in the infirmary although the author doesn't draw it, it is implied.
