I think somebody has already mention Olimpos, I second that, as long as you're fine with androgynous (like really androgynous) characters.
Banana Fish is pretty different, and I don't think I've ever read anything like it. I read it a pretty long time ago though, so I don't remember perfectly, but I think it's just a little violence.

So many of these aren't unusual at all and are typical otaku shit, way more than you'd expect, ugh...I guess everyone must have a different definition of mainstream.
Anyways, seconding Kokoro no Kanashimi and adding Kono Sekai no Owari e no Tabi by the same mangaka, Space Ship EE, and Travel.

Everybody should read Ryuuguuden, because it's fantastic. Paperakyu by the same author is also worth reading.

Now, if you'd be satisfied with finding some of the least-commonly read series (available in English) among the MU community, you could try to find a copy of the following, all of which are offbeat collections of oneshots currently on fewer than 15 complete and custom lists:
Manga
New Engineering (same author as Travel, recommended above by spikebaby)

You could also try any of the oneshots by TSUGE Yoshiharu, or perhaps the first volume of Ax: Alternative Manga.
