Sorry, I just have to say that this thread makes me quite happy.

I am a huge fan of suffering bishies. (I like all sorts of suffering biggrin ) /SHOT DEAD If you have any recommendations of any genre, I'd love to hear them. I'm afraid there aren't very many out there at all, and among them, there aren't many that are awesome. Here are a few that come to mind. Some are better than others.

Diamond Dust (KANG Hyung-Gyu)

Spoiler (mouse over to view)
The male lead has brain cancer.

REC - Kimi ga Naita Hi - I didn't find it very compelling, but it's not bad.
Nabari no Ou (It's not so much a matter of hiding it as it is a matter of just not talking about it.)
The anime for Hakuouki Shinsengumi Kitan also applies. The characters are modeled after the members of the Shinsengumi, one of whom was Okita Souji, who had tuberculosis.
Heaven's Door (HONGOU Ranko)
Papillon - Hana to Chou I seem to remember stomach ulcers. biggrin

You can also try your luck with tags like "imminent death," "terminal illness," "cancer," "heart condition," "disease," "illness," etc. There are also tags for "sickly character," "sickly uke," and "sickness."

Sorry I couldn't be of more help. Happy hunting! I'll post again if I think of more.

AHA! Thought of another one. Well, you might have already read it, but in Boku no Hatsukoi wo Kimi ni Sasagu, the male lead has a heart condition. He cannot hide that, but he does hide some of the unsavory details from the person he cares about.

Four Leaf Clover oneshot in Cello Mellow
Flower That Blooms In His Garden in Kare no Niwa ni Saku Hana
Kyou no Kira-kun
(Sorry none of them are actually from the sick person's point of view, but they are the secondary main character.)
