What is the best way to translate a dialect or accent of the original language that is not the norm (e.g., Kansai-ben)?

Inverted Nipples Yaoi ?

So I remember reading this yaoi manga a while back where this guy had inverted nipples (?) and another guy "volunteered" to help him fix it by sucking them out. There was only 1 chapter when I last read it though so that's all I know of ><;;
