So basically what I am looking for is a manga or manwha were the main male character has some sort of physical hidden scar. Like maybe his whole torso or something and he try's to hide it but the main girl someone sees the scars he has on his body and freaks out or gets worried or something along the lines of this. I do not care what genre at all just please if anyone has anything like this could they please tell me of such a manga.

I think the male lead in Full Metal Panic fits.

Sounds like Vash the Stampede in Trigun.

Unmei no Oaite wa? best fitting
Omokage o Motomete
Midori no Me

I hope you don't mind the 1st two are a Harlequin novel based. The third is a oneshot.
Also there is a tag of Scar/s that could help you out, but you would have to read a the description, and hopefully it'll give a hint if the male character is the scared one or not. I hope you find my suggestions fitting.

Meteor Methuselah to an extent.

Wait whatconfused? Normally I wouldn't say someone's suggestion doesn't fit, but a physical scar in Horimiya? In the male lead emotional scarring sure. Tattoos, and self done ear piercings yes, but scars confused did I miss something? (To my knowledge there are no physical scars in this manga)

According to the description it sounds exactly like Trigun when Meryl Stryfe unexpectedly saw Vash's bare chest for the first time.

Honestly, I don't reallly think a piercing was that much of a scar. But other than that, Horimiya's pretty much fit the bills. biggrin

- Physical hidden scar (technically). bigrazz
- MC tried to hide it.
- Main girl sees the scars and get worried.

Well, it's basically a romance manga, so the scar's thing doesn't revolves around that much. biggrin

I wouldn't consider a tattoo a scar. I mean there is a tag for scars, and there's a tag for tattoos because they are different.
I don't want to make this into a debate of is a tattoo considered a scar. It's up to zixiz if he thinks it's fitting.
I will give you that the female lead was shocked when she saw them, but It's just one chapter. She seems fine shortly afterwards. Where if it was a scar every time the female would see said scar it have almost the same reaction, just to a lesser extent. and yes, Horimiya is a good manga worth reading, but I still feel it doesn't fit the OP.

Here I thought Meteor Methuselah was stretching it a bit.

You could find something like that in Deep Love: Real.. but it takes a few chapter to reveal the scar

I think Shuumatsu no Laughter fits, but if I say why, I will spoil you.

All in the shoujo genre (although, knowing main male MC has a scar can be considered a spoiler):

Penguin Brothers
Platinum Garden
Kara no Teikoku

Non-shoujo, so, not a girl discovering a guy's secret: Pandora Hearts
Spoiler (mouse over to view)
Oz discovering about Gil's....
