A story that begins what the majority of characters introduced thus far being killed off. Kingdom gets betrayed by its allies or entire family being assassinated by rival. Quest for revenge begin, plus if if significant amount of companions drop dead along the way. A historical setting works too.

I don't know of a manga like that but the novel series 'Song of Ice and Fire' is certainly like that.

Vinland Saga has the historical setting, and a ridiculous number of character deaths (although not in the beginning).
Fullmetal Alchemist involves a genocide, but there isn't much information about it until Volume 15. It does drive a lot of the plot, though, and there is a great deal of revenge-seeking.
If you enjoy beloved characters getting horribly murdered, I second the recommendation for A Song of Ice and Fire. If the length of the books is off-putting, the TV show is also excellent.

uhh, yeah, A Song of Ice And Fire (books) and Game of Thrones (tv series from those books) fit perfectly. Love 'em.
manga...? ummm, well...massacre...
ok, 1st thing that comes to mind is A Kiss to the Prince cuz the main girl's whole family was murdered due to jealously for power etc & branded traitors, but other than that...revenge? not really, certainly not from her, but still might try it, it's not long anyway but in my opinion it's kind of not all that uuuum serious, certainly not if compared to GoT, but still might try
maaaaaaaybe Ayashi no Ceres there was some killing i think...nope, no massacre or was there, nah, forget this one
oooh, Chrno Crusade fits pretty good i think, cuz well, not quite at beginning but there was a massacre of demons & other characters die, yup, this one just might be it & there is a side plot whatever you call it that fits 100% - whole family murdered, revenge -, its part of the main story anyway - read it
um Immortal Rain also fits pretty good, there were certainly some gruesome deaths & mass murders
well Vampire Juujikai fits as well, there was most certainly a massacre, others die along & a need for a revenge - it's a fantastic story anyway, if you know Code Geass & liked it, then know this ones just as great, i love those genius minds
oh yeah, maybe you shoudl watch Code Geass: Lelouch of the Rebellion worth it anyway
and that one makes me think of Guilty Crown massacre? yeah, deaths? aha, revenge? i think there was?
back to manga...massacre, massacre, fantasy massacre...
zomg, how could i have not thought of Pandora Hearts i'm ashamed, truly. Massacre? Check. Deaths? Check. Revenge? Check. And all of it multiple times. It's a definite one, thou they were dead when the story began but not sure it was thoroughly meantioned at the beginning, a bit later. You (should) must read it.
Can't think of anything else at the moment
P.S. maybe Scarlet Palace works as well cuz there are deaths, but i don't think there was a massacre, in recent chaps whole family was banished/their power taken away i think, revenge? yeaaah, probably. Don't know, maybe could try

Akame ga kill! is a fantasy setting. The focus is an assassin squad during a revolution, so expect character deaths.

Houshin Engi - quite few characters die and the is a lot of "civilian" massacre
Outlanders - sci-fi not a fantasy but fits your request perfectly
Shin Angyo Onshi - death, genocide, apocalypse, a standard dark fantasy setting
