Stuff like Berserk, Magician, Ares, Claymore, Ubel Blatt, Magi, Id.
Furthermore, I am using Medieval in a very loose sense. By Medieval I really mean in time period without guns. Preference to a more darker/mature manga, but, not required. Also once not required preference to a longer series.
Stuff I tried, but, I dont really like is: Shin Angyo Onshi, chronicles of the cursed sword, and some other ones I can't recall.

While lacking a bit in the fantasy element, I would recommend Vagabond and The Ravages of Time.
The second is a bit hard to get into because of the plethora of characters, but if you can it is well worth it.
