I am right with you on this. I am constantly looking for this type of manga. I will be poking around in your public lists to see if there is anything I might be interested in that you found. Here are a couple that I thought you might like and didn't see on your lists:
Tsuyokute New Saga - This is a classic fantasy "defeat the demon lord" thing, but the hero already knows what going to happen. He defeated the big bad with everything he loved and wanted to protect gone, and then goes back in time. It kind of feels like a second play on a video game.
Red Storm - A fighting master from another world appears in in a fantasy world filled with monsters. He is taken in by a tribe that hunts the monsters and ends up training the leader's young son.
I saw that you read
Immortal Regis, but I couldn't find the sequel
Cavalier of the Abyss. I have found it quite enjoyable.
Saver - Female Kendo fighting captain is pulled into a fantasy world. She dresses as a boy to protect herself as she tries to survive. Its labeled a Shoujo, so it has a different type of art style and there is a bit of romance (Not that much). I think it is a bit slower paced though. female lead is strong.
Basara - Also a shoujo about a girl in a fantasy world. Her twin brother is suppose to be the hero that saves their tiny village, so the king comes in and kills him while he is still young. In order to keep her village going the she takes her brother's place and leads a rebellion against the evil king. Less focus on martial arts, but still has lots of battles and fighting.
Threads of Time - One of my favorites. Its a little darker and does have the tragedy tag (but the ending is really well done and not depressing). Guy is dragged into the past by a "vengeful spirit," where he tries to get stronger to survive and find his way home. This one feels a little confusing at times but it comes together well. This is more of a historical.
Drifters - Famous warriors from different time periods find themselves pulled into a fantasy world where they battle to conquer each other. Done by the same author/artist as Hellsing, but so far it has not been as gory. Updates have been slow.
Ryuurouden - Two kids get dragged into the three kingdoms time in china. Though it is a historical fiction there is a lot of both fantasy and martial arts. This is a very long running manga with 100+ chapters

although i really hate waiting monthly for good manga to come out lol
just sad that releases are weekly and not fast like UL on Japtem)
