I detest characters that don't have high self esteem! Sometimes it's cute playing damsel in distress, but it gets annoying if it's done rather often. I'm reading Gokusen and that's what I like about the main female character. She's strong and doesn't have does moments of weakness like acting dumb and stuff like that. It's sooo stereotypical!
I don't like that they are somewhat simple-minded and every time there's a little argument with the main male character, the female char. goes like "he hates me now". I'm sorry but hate is a strong word. They shouldn't just throw it around like that, why can't they be more confident.

Hmm... most of the scenes you guys mentioned are quite cliche but I don't really hate them, just take it as part of what makes it shoujo. If you were talking about the worst kind of cliche, it would be when the ugly girl gets the guy. Now THAT'S what I call completely unrealistic, but you get that kinda plot all the time. Come on, what are they trying to do, make us girls feel relieved that we aren't perfect but can still land a guy who appears perfect? Puh-leeze. But then again, it doesn't really annoy me, since I'm used to it already (read too many shoujo manga to get irritated ^^")
I do find that manga where the girl gets raped and now she's like a complete weakling etc really gets on my nerve (Hitomi Genki Kingdom/Kiss in the Blue). I don't care how realistic it is, but putting some girl who just sits there and cry 24/7 is more than I can take (so far I think those are the type that I absolutely will not finish). Oh, and teacher-student love stories are just disturbing. I don't know why, but they are. I guess it's the maturity issue...
I agree with the student - teacher relationship. Maybe if the ages aren't sooo far apart and maybe if the student is already 18 and the teacher is like 21 lol, you know, not so far apart...but then if they both are in the same environment, now that's just not fair and not appropriate either.
