I would strongly recommend Flowers of Evil if you don't have anything against an incest theme.

Try these topics... I'm sure there's more if you look hard enough.

manga that will make me cry
A tragic romance...
Romance with a bad/tragic end

Naturally, some of these topics will have spoilers, because it's telling you the end will be tragic, but that's what you're asking for...

Boku no Hatsukoi wo Kimi ni Sasagu
Bokura ga Ita
Deep Love - Ayu no Monogatari: Read the prequel and sequels as well. Serious depressing stuff.
Watashitachi no Shiawase na Jikan
Zettai Kareshi

The last chapter of Mademoiselle Butterfly made my cry.
I don't tend to read sad mangas I don't like crying no

World Embryo--its complex and not as simple as the thing that you want but you will cry for sure...the latest chapter was really sad. The protagonist is so human, so selfish that you feel like yea some people would do that

I second Bokura Ga Ita & Zettai Kareshi & Mademoiselle Butterfly .

I know a few more but can't come up with them atm.

For tear-jerkers I always say Candy Candy; it doesn't follow what you described exactly but there is something very similiar with one of the relationships.
Spoiler (highlight to view)
At the begining, Candy's first love dies in front of her. Towards the end, Candy is seperated from her one true love because he has an obligation to marry another girl because she saved his life and lost the use of her legs because of it.

Eden no Hana sort of. It's gender-reversed though, but it's really good. biggrin
