What is this manga relation with City Hunter?
Is it just spin off/alternate telling, or is it a direct sequel of City Hunter (maybe with some time jump)?

It's actually a direct sequel from City Hunter which told us about Ryo Saeba and
Spoiler (mouse over to view)
Kaori's heart which transplated to Angel Heart's protagonist,
Shan'in.
