Monster, all the way, 100%. Although there are a couple other decent series here, Monster is way significantly more complex and better written than almost any other manga I've ever read.

Had to go for Gantz, though I feel uninformed regarding many of the others since i haven't read them. Hmm, almost kept me from voting.

Monster. That manga is pure brilliance. Gantz is also very good, but Monster > Gantz IMO.
