I'm not sure if its realistic but, Yamada and the seven witches is a hilarious body swap manga.

I don't know if it's exactly what you're looking for, but Ame Nochi Hare (Bikke) might work. It's not a "body swap" as in two people switch bodies, but the main characters' personalities do change slightly (and more so as the story goes on) when they turn into girls...

The most realistic body swap i ever found... strangely enough... was actually a hentai...

Huh... the idea of body swap is already unrealistic... Personality changes due to new body/hormones? The closest thing is Boku to Kanojo no XXX. To be exact, it's not really personality changes, but the new bodies fits their personality better than the old bodies. Like, feminine boy becomes feminine girl.

Another one might be Jinzou Shoujo... Other than that, usually the personality stays the same.
