Interesting request. The person you've described reminds me of a couple of Miyazaki heroines. I believe Nausicaa has a manga, so you can go check that out. There's a page for it on the site called Kaze no Tani no NausicaƤ.

Superior has two protagonists, the demon queen Sheila and the hero Exa. Exa is pretty close because he's willing to put his life on the line to defend any "being" (be it human, monster, or sentient skeleton-things) against evil. He's also very much a pacifist, and much of the time his life is only in danger because he refuses to kill in order to solve his problems.

Other than those, I've only really come across such protagonists in video games (Okami). Usually, in manga, it's the side characters who fit your description, and they tend to not last very long.

Most other self-sacrificing main characters are concerned chiefly for their loved ones and far less altruistic in nature, but still endearing. I really like Jin from Zetman - he puts his life on the line for others (even complete strangers) multiple times. He mostly keeps to himself, but you can tell he really cares about people, such as in the way he enjoys helping out the slum community by hosting dinners.

Kaneki Ken from Tokyo Ghoul also fits your description, sorta, but as the series progresses

it's hinted his obsession for protecting others is based off of a fear of loneliness and isn't quite so "pure" in intention

Other mangas with MCs who constantly put their lives on the lines for others are Basara and Bleach, but their lack of self-preservation usually only kicks in when someone they even vaguely care about is in trouble.

It's originally a VN but got adapted to manga and anime.
manga is quite fail but 2015 remake anime is quite good

This is basically the character profile for Vash, the protagonist of Trigun Maximum.

I think Ginji (one of the two protagonists) from Get Backers is like this a lot of the time, too. Not to the extent that he doesn't care about his own health, because he definitely does, but to the point where he'll put himself in grave danger for someone who isn't a friend/family/love interest or even much of an ally.

Nao, the female MC from Liar Game, who regularly tries to save enemies at great cost to herself.

I agree with macgyvor; Fate/Stay Night is definitely ideal. The protagonist has suffered a hard past and believes in looking to the future and never to the past. You'll see he sacrifices himself in fatal situations and that he never denies to help people even when their intent is ill.

Tiara will definitely not seem like one, and there's only one or two cases in which the soft, sacrificial side of her is shown....and they only come later in the manga. I personally love this manhwa but I don't know about you, interesting request indeed smile wink grin
