What is the best way to translate a dialect or accent of the original language that is not the norm (e.g., Kansai-ben)?

I'm new to this website, and I don't know how it works, so it contains sexual content.

So I've been going through the Yaoi manga, and ther seems to be a lot of smut with the sadist and masochist theme (which I enjoy), but I've never seen anything with a female masochist and male sadist.

And I don't mean a female like, "I'm so shy but this guy made me enjoy sex." I mean, Blood Bank kinky.
Does this even exist?
