In FFXII.
I was in this ruin (I forget where exactly, there're a couple of ruins) but in there, it was really, really, really dark.
I eventually discovered the reason I kept on running into Zombies [and couldn't find the exit] was because I was basically running in circles, and had to use to map to get out.
Hmm...in Kingdom Hearts I, you know all the trinities? Well, I missed one of the red ones -In Halloween Town- after I beat Boogie, his castle was destroyed, so I never got to get that Trinity, but then I started the game again and got it that time around.
...Yeah, they're pretty dumb, don't you think?

Not save right before a big boss battle, lose, and have to start way back somewhere which is basically starting over.

It was a while ago with FF8. I got up to the battle with Raijin and Fujin in Balamb(town) and was annoyed as to why i could not beat them.
The reason was that i had gone through the entire game to that point without using the junction system at all, even though there was a very extensive tutorial at the start.
Stupid stupid me.

The most retarded mistake of mine:
Wolfenstein: Enemy Territory - new map, so I thought there's some seconds left till it starts for real. At that time I chose a panzerfaust as my main weapon and happily watched half the team trying to squeeze through a small exit from our respawn point. So, with a beaming smile on my face I blew up like 5-7ppl in one go, myself included. Then, a moment later noticed the texts saying I've killed my teammates. Goes without saying that I quickly left the server
Number 2:
I don't remember why, but somehow I didn't know how to start the first mission in FF8. So I've spent like 30 something game hours just lvling Squall and Quistis and the 3 first GFs. At least I didn't have to bother with lvling for half the game afterwards

Playing WoW I've been walking/jumping in Thunder Bluff ( Tauren main city ) and reached one of the bridges there. Well, somewhere just before half the way through it I jerked my hand ( the one holding the mouse ). Guess what? I watched with my mouth gaping as my character jumped off the bridge.
I suppose I was probably one of the first suicide jumpers in WoW . And completely accidental, too ( I was like "Man, that was such a dumb thing to do" ).

Lol, some of these entries don't make me feel as bad for some of my epic fails.
For me, I was replaying Pokemon FireRed on my computer and had put 100+ hours in it. Reformated my Hard drive and thought I had back-up everything everything.... except it seems the save file for my Pokemon game.
Also a couple of times, I would walk away, come back and tripped over the power cable to my Sega Genesis , deleting all progress for the day

Got "whited out" by a level 3 or 4 kricketot in Pokemon. It used bide and I wasn't paying attention and had only one pokemon with me.
Kricketot is the weakest pokemon of all, too. It's stats are worst than magicarp. xD

Not one that I made, but my brothers had the habit to overwrite my date on my game. That always annoyed me, since I usually spend 40+ hours on it, and that isn't easily achieved back...

I had over 20,000 souls collected in Demons Soul and because of a single mistake in blocking I lost them all.

Not the biggest, but one of the only ones I can remember.
In Digital Devil Saga 1, when you're climbing the Karma Tower, a ways up you go outside and run along an outer rampart. At the end there is this flying boss that is just the most absolute rape thing ever.
The best way to beat it is to use debuffs on it since it doesn't have the skill to remove them. I hadn't developed any of my characters with the proper Mantras since the other skills in those Mantras were garbage. I ended up spending, I shit you not, probably somewhere near 5 hours grinding myself stupid on that rampart so I could earn the atma points necessary to get the mantras so I could kill that boss.
I knew the enemies well enough that I could kill any combination of them in 1 turn, every time, if I didn't whiff an attack or get jumped.

You know in FFXII when you go out to the desert for the first time and you're level 3/4 and you see this big T-rex and think: "Heck yeah! I can take this mother down!" Only to have take half your life away with one attack and have Vaan run like a nancy to that dotted like to save you life to only be killed right in front of it.
I even saw a 3 panel about that in a gaming magazine (which one was it again...). Made me feel slightly relieved that I wasn't the only idiot who thought they could take on a T-rex at level 4

In Kingdom Hearts (1) one of the MANY time that I tried to beat Sephiroth I was almost there... his HP bar was on green... but he used that strange, annoying and stupid power of his that left me with 1 of HP and zero MP... (no I didn't hit him to avoid that...) and I couldn't use my elixir because my finger would slip all the time... I panicked and ran right in his direction... Of course I died
In Call of Duty... I don't know wich one... I was playing with my brother... but he didn't teached me how to play... So I decided to test all the buttons, in front of a wall... >.< one of them was the granade button... So it hit the wall and went back to me =] And so I killed myself (I did that many times in the game... That's why I don't play war games... I only kill myself)
When I used to play Tibia a lot I was a real noob when I wanted... So I was alaways trying something new... and stupid... Once I was on lvl 12 and tried to kill a DS... also I was a sorcerer and only had that stupid first wand... Obviously I died... But I returned to recover my stuf and of course the DS was still in there... (why wouldn't he be?) So after I recovered my own loot I kept runing and atacking the skeleton until I killed him... He had no loot, and gave me almost nothing of exp ...

i remember one time i had been playing HL 1 for hours without saving and then i accidently saved half a second before a grenade detonates and kills me, not fun to see yourself die every time you load the game, tought me a valuable lesson save often and in safe places.

You could have modded in invincibility, then removed it after you "recovered".
