does anyone know the title of the manga thats fits this description, any help would be greatly appreciated!

Uke used to be small and cute. I think the seme had to move somewhere so they were seperated for sometime. when they get reunited seme doesn't recognise uke.

i think theres an extra for it where uke keeps making seme bento for lunch, and seme gets worried about his weight.
