Main character can sense or followed by supernatural or has some sort of powers. MC has been having issues possible feeling sick or something and collapses and soul/spirit is in danger of leaving his body i think is seme/partner who has been following MC for a while and helps save the MC by seal MC spirit/soul back to the body MC may have been reincarnated or something the one that saved him/following him has known who he is

Katsuragi-san wa Tori Tsukare Yasui is what I thought of first. Sounds a little different but memories get distorted sometimes so I figured it was worth a try. Either way it's a good manga.
