Yeah I always took them to be chosen by Aizen, maybe that's just me.

Well, there was sort of a hierarchy, arrancars were mostly incomplete before Aizen messed with them, but there was the 4 tiers of hollows. Basic hollows and then the 3 tiers of giant hollow. Granted there was no command structure if that's what you're saying, since it was just strong kill the weak (as explained with grimmjaw's past).

I think it was something like Aizen couldn't create a complete arrancar without the Hougyoku. I think he stated that all that what he had created before were incomplete versions of Arrancar.

Maybe that Hollow was the boss first, and made the hierarchy.

the huge eye thing was what I want to know about, it was there in the beginning of the story and appears every time the hollows tear through dimensions. Plus he seemed to be calling the shots of the hollows long before aizen and the espada, so unless that guy was turned into an espada and we dont know yet, really wonder what that is.
