took it but i think you should change african american because not everyone that is from the African diaspora is american and add an option for hispanics. Also, is pacific islander included with asian or native american? maybe i missed it

I used generic questions and responses from common demographic surveys plus input from the department to built this survey. The consensus was these were the best way to word the questions and responses.
Hispanic was an oversight and I apologize for not including an option reflecting that community.

Done, and just like everyone else, I'm interested in hearing about the results when you're finished
