Getting blood work done. I have to do it every few months, and its not really the needle that bothers me, but the thought of something that was once inside my body is being taken out just disturbs me. dead ugggh.

Uuh, yeah, that finger-licking sure gives me the creeps...
Scratching on paper and such, as mentioned before, as well.
And people randomly touching my exposed extremities...major chills!

My worst: someone putting his chin at the place where the neck meets the shoulder.
I can't stand this at all, my whole body shivers uncontrollably. Funny thing is, I have no idea why or when it started. Just found out I hated it when my little brother rested his head while looking over my shoulder, few years ago. (I love that guy, though...can't be his fault^^)

This. Not because it freaks me out but because my neck and shoulder area is really sensitivie.
Nails on chalkboards.
Fork or knife scrapping across a plate.
When I'm bout to do something I really don't want to do.

I get the chills when I hear or even remember the sound of nails on a board or a fork against a plate or something.
Needles REALLY creep me out. Each time I see them, I feel like crying/groaning/running away.

When somebody starts peaking/scratching themselves vigorously.
And I don't mean those coy gestures of those who're trying to calm a sneaky itch, but the horrible I'm-alone-in-the-world sort of thing.

A good example:
I was riding the bus ones, then I see a women suddenly thursting her finger into her ear and shaking it with all her might.
While keeping a calm face at that.

Wow. The inside ones with legs that spread out flat against the wall really give me chills. Can't believe I didn't think of that. The bigger and hairier they are, the more I think of them as "animals" so I'm fine.

When I get a fountain drink, and the straw scrapes against the hole in the lid. <<That one's really bad.

The thought of touching my friend's bald spot... There was a birthmark there and his mom tried to remove it, and it also removed his hair.. so its like an unnatural bald spot coz its not shiny like all bald spots should be. sad kinda look rubbery..

It looks really gross too.

When a girl licks the back of my ear. Also seeing a lot of blood. Nails on chalkboard and similar sounds, I actually feel those noises in my teeth.

When I was younger, it was the sound of a chair's feet scraping across the carpet in the classroom, pencils across paper, and the feel of certain kinds of fabrics... don't know what it is just that it's an unpleasant texture.
