Well when I think of realistic first times Hana wa Saku Ka comes to mind. It's an on-going series though, and they haven't quite reached the climax yet (not that I know of, I've kinda lost touch with it). However, from what I remember they do explore a bit (foreplay and whatnot) and they are very meticulous about the preparations. It's a slow paced romance, if that's your thing.

One of the few mangas I've read where ahem.. special preparations that should be done by uke are mentioned was Electricity Delusion 😃

Just wanted to point out that there doesn't have to be blood or a lot of pain for it to be realistic. If there is blood from anal sex, you are doing something wrong. There does need to be preparation though, which most manga seems to leave out. Anywho, if you don't mind djs, I just read Free! dj - Sonogo no, Orera where a couple who has trouble having sex end up time traveling to the future, where their future selves teach them how to have sex.
