Oh I just remembered the best mistake when I was at my friends.
She was playing Uncharted (I can't remember which one) and you were SUPPOSED to run backwards while shooting a van.

Well. She forgot to run backwards and ran into the van and got run over.
It was fairly graphic.

We looked like this: O_O O_O
And then we laughed really, really hard.

Playing killer7, the second fight with Ayame Blackburn in 'Ecounter Part 2'.

Before the boss fight, one of those heavy armoured Smiles killed me as Con, so I was playing Garcian in order to revive him. Accidentally took a wrong turn in the maze that is Curtis' mansion, and ended up walking into the fight with Ayame.
I thought "Huh no bother, I'll just switch to Mask de Smith", but turns out you can't switch characters mid-boss-fight. I was stuck with the weakest persona, the one that wasn't intended to be used in killing at all.

Needless to say, it took me the best part of an hour to kill her with that shitty silenced pistol. 😐

oh and when i was 5 i deleted my cousins donkey kong country saves, they had all the bananas

I was playing this shooter game (America's Army, maybe? I'm not sure), and I accidentally set off a grenade while in a group with a bunch of people. I killed around 10 of my own teammates, AND myself. >>>

Bought/Played WoW almost since release.

Leveling to 80+ in FFVII before the whole Aerith x Sephy event >.> didn't change the outcome fyi.

This was before internet for me.

I did not pay enough attention to the cloaked figures in Final Fantasy VII to get Vincent Valentine... and the last save point I had before I talked to them was right after I left Midgar.

Oh, how that pissed me off. I wanted that character so much that I just stopped playing.

Overleveling all my top tier demons to 99 in my attempt to defeat Lucifer only at my second playthrough at Shin Megami Tensei : Devils Survivor. And i mean all of them even the ones i pick at mid 50s and 40s level. Thus making my third playthrough incredibly boring and easy, also seeing all my demons leveled to 99, with good stats and skills make Lucifer seems like a lame demon with poor and below average skill and stats. Hence i stopped playing, although i really am curious with the other ending, the only ending i play was Yuzu's and Amane's ending.

soft resetting for over 18 hours on pokemon heartgold to try and get a shiny Ho-oh (and not getting it so far.... BUT I WILL CONTINUE!!!! I MUST GET ONE Q.Q)

I stupidly went to forward to Halgita before I learned Mare Lunaris again. Augghhh, now all my characters keep getting turned into vermiforms due to the lunar rain, FFFF----!.
So yeah, I keep on dying now, it sucks.

I think you have a really, really slim chance of getting the spear in a mine anyway, but regardless, you really don't need it. I had none of the best weapons and I still beat Yiazmat on my first try with an insufficient amount of mega/elixirs.

Haha, it started out as nothing more than having a look at him, but after those first two hours, I became a lot more invested in seeing him dead.

Disregarding the way I played games in the past (Running from most random encounters, etc.) the most recent mistake I did was deleting my Shadow Hearts-saves and accidentally removing my SH: Covenant save (1st disc end) as well.

...That second playthrough through the first disc was ridiculously boring.

Me too! It was the type of game where you could get more unlockables after completion if you saved less frequently throughout your gameplay. I was so into it that I totally forgot to save, and once I got killed it was like "Wait a minute. Holy sh**....."

I felt so stupid 🙁

A long time ago, my brother was complaining about how he couldn't capture Suicune, Raikou, or Entei in Gold Version, so, as I was the one who owned Crystal Version, I traded him my Suicune so he could get its Pokedex info. However, while we were returning each others Pokemon, the battery on my Gameboy died. Needless to say, I lost my Suicune and had a clone of my brother's Red Gyarados. (The only good thing was that I got a shiny Pichu from the Daycare when I started the game all over again.)

I remember playing Super Smash Brothers Melee one time and jumping off the cliff. Needless to say, I wasn't playing Kirby or Jigglypuff, so I died. (I usually play Kirby, though, so it could be fail muscle memory...)

Playing Kirby Tilt 'n Tumble on the GBA SP or DS is... quite the challenge, might I say, considering that you're tilting in the opposite direction you tilt the system. XD

I was playing one of the Shugo Chara games last summer and got pretty far, but because I was playing it on an emulator that doesn't really support saving, I lost my data whenever my computer turned off. (If anything, I learned how to say Sunday in Japanese from that. ^^ ; )

I also played Platinum Version in Japanese once. Let me note that I'm nowhere near fluent in Japanese, so when you had to talk to the clowns to get the PokeGear, I had no idea what they were saying, so I just randomly selected items until I got them right. ^^;
