I just finished the series....and I have to say..the ending is very vague
it ends with Takuma going to surgery after getting married, but he leaves a will for Mayu saying that his true wish is for their first love to bear fruit. The manga ends while he's still in surgery...whether he lives or dies is up to your imagination. :)

He dies. People don't get the ending because is more or less abstract, but he is seeing her future with children as his vital signs are fading. And the last panel of the manga shows how his heart readings have stopped.

does mayu not die in volume 7 chapter 34 then i was CRYING DAMNIT :'( SADNNEESSS someone anyone can you fill me in on that? or did she ! just get really badly injured???

Nooooo!! SOB. But I also think that a sad ending would be more fitting than the "happily ever after"... oh, but how I wish!!! how i wish

He doesn't die because in the actual volume 12 at the ending the author thanks everyone and then she says:
<Also to the girl that came crying in the signing sesion in april at sapporo and told me: "Please, let Takuma be happy"

Technically, he doesn't have to live to be happy. A person can die happily, too..like knowing the one they love is happy, for example.

Maybe he doesn't die -- after all, he's having open-heart surgery, and for that you have to stop the heart beating (this is why you need a heart-lung machine) so that the surgeon can work on it more easily.
So, his flatlining could simply be the starting phase of his surgery; and the last scene where he's in a clover field with Mayu and their children could be a picture of his last conscious thoughts.
...Of course, this still doesn't tell us conclusively if he survives the operation or not. 😛

The ending is very interesting and of course I think it was intentionally left open-ended. In my opinion though, he was dreaming about a future with Mayu and his children as his heart flatlines, but I think he must've survived as his narration ends and the dream becomes a reality at the very last panel.. as if the artist is trying to represent the transfer of his body out of the operation room and into the present. After all they've been through, I really think they deserve a happy ending.
Did anyone see the live action film though?
The film was more depressing as Takuma doesn't survive the operation. I hope this was just the director's own interpretation of the manga as I'm sure leaving a film without closure would give it bad reviews.

^^^ thats what im thinking. I think he made it through. the white cut off really wasnt a death sentence..
