NTR is a good excitement component but not limited to sex scenes. It's been used in novels and comics with adult romance background, many closely related to real life. The term NTR become more popular after being ported to some stupid AV / adult games which made it a fetish with frank sex scenes in front of previous partner.

But extra-relationship affair is not that rare in real life although not wanted mostly. As a component for plot development, it's definitely alright. There only are a few H manga/anime which really have plots, so the chance of getting good NTR is from seinen or josei works. NTR can be seen as spice or explosives, it can trigger positive reactions like closer relationships, and negative ones like murder.

For me NTR Vs Vanilla..
As far as i concern in every story their is always the aspect of being cheated. Like Their was an hentai i watch its about a doctor(main) that uses his profession and giving pleasure to his clients/ patients mostly girls. Well, good question. Not sure, overall I would say, Vanilla, just because Sex on consent ist way more beautiful to watch and, though as for all hentai depending on the artwork, satisfying.
Anyway- sometimes I am longing for some NTR. Not especielly the messed up style of Naked Apron (I liked some works, though).
I am talking about somehing like Yuzuki'N's Another World. I'd have to side with vanilla.

Most of the NTR I've read have been pretty cruel to the wife. Like she gets blackmailed into cheating, or there's a rape-mind break scenario. I think NTR has the possibility to be more interesting than Vanilla if its consensual and more realistic. But the majority aren't realistic in any way.
