I don't think it is what you are really looking for because it is just a one-shot but Miniature fits almost all of your requirements.

midori no hibi is similar but the roles are inverted, mc is normal, heroine is shrunk.

Well, there is Chou Dokyuu Shoujo 4946 which is similar. He is normal size but the girl is a giant, so resulting in her carrying him around.
