A manga taking place in an alternetiv realety or a dream world where the climax of the story is that the protagonist have to make a choice "the blue pill or the red pill", an example would be Houkago Hokenshitsu or Coo no Sekai thu im less sure about the last one since it was a wile sence i read it.

The entire Ghost in the Shell franchise has elements of this - whether a character chooses to believe or not.

Have you seen Paprika, the animation? You will LOVE the hell out of it; it's exactly what you're looking for, and more.

Homunculus is very much about whether the protagonist can CHOOSE to accept this new reality that his trepanation reveals to him... and the consequences for him if he does.

Ultra Heaven, recently recommended to me on another thread. And boy, was it worth it. <3

Me & the Devil Blues - so brilliant you'll feel like you left your head behind. The Devil offers a guitarist a choice - not exactly red-pill/blue-pill, but certainly a choice between living two very different kinds of lives/realities, albeit in the same world.

Alice in Heartland might suit you, given the whole Wonderland theme.

I'm not too far along in Amatsuki, but it might head that way...? I mean, towards a choice of some significance - but I can't be sure.

Paprika Love it to but she doesnt have to shoese between dram and realety.

Homunculus Ultra Heaven Havent read far enuf to make a judgement on them.

Alice in Heartland Didnt finish it, is it worth pickin up agen?
