There's a huge difference. In smut, there's sex but no visible penetration. And the sex scenes are usually short. Like some said, it's like movies/TV shows where there must be a sex scene.

Not just shoujo and josei, there's seinen smut too, I think. It's not labeled smut but usually ecchi or mature to differentiate it from female targeting manga.

Hentai usually have close-up pictures of genitals and penetration. Not to mention all the body fluids everywhere (they can even be exaggerated to become a fetish). Hentai is more about fetishes than plot. That's why you will see a lot of weird/physically impossible stuff that is not for everybody. Though there are some hentai that are close to smut (some doujinshi I read and some of the most softcore hentai) but they are not the majority of hentai.
