I read a couple stories with this kind of "action" but I can't recall the stories names. I suspect I saw something in one of KANO Shiuko's manga (maybe Steppin Stone, Kawaige or Otonage) but I'm not sure about. She makes awesome smut.

Don't know any manga but I guess you could find clips of or download the game Hanamachi Monogatari. Lots of types of double penetration in it. laugh I am not fond of the story or the art style (no problem with TAKATSUKI Noboru).

This doujinshi probably has: Kateikyoushi Hitman Reborn! dj - Gokunyanko Okawari!!. I am not quite sure though.

Um, I'm gonna suggest the fifth chapter of Koisuru Omocha. Or for a slightly different take on double penetration, you could go for the first chapter (where there's already a toy inside).

I can recall 2 collections of oneshots that have it (not in each story) -
Himitsu no Kemonotachi
Sokonashi Freefall Romance - be prepared, the stories are so weird that "double penetration" looks like normal to compare it with other things the guys do smile
