Does anyone know any hardcore/smutty yuri manga's that are directed at lesbian women? Like the female variant of Bara or something? Or even just hardcore Yuri that can be read by women without being horrible unrealistic?

I think any yuri/hentai can be read by lesbian women. It depends on what you expect from those manga and what you perceive to be not unrealistic. You could be a bit more specific, like saying what you want and don't want in yuri. Do you mean something like body proportions?

Sorry, I'll try to be more specific;
- Realistic body proportions - No giant breasts, please.
- Not two whimpering crying messes (One is somewhat okay, but two is just.. no)
- The sex needs to be atleast somewhat realistic
- No futanari, and no "two lesbians getting it on for a guy two watch"
Basically I want something hardcore that isn't blatently misogynistic or 100% catered to boys like it usually is in hentai.
Something like Shoujo Sect is exactly what I'm talking about.

not really "hardcore" but you'd probably like Hanjuku Joshi. There is sex but it's not overly explicit and just overall really cute.

If you want something that is specifically directly at lesbian women, then queer publications would be the place to look. Carmilla is the only hardcore one that comes to mind; they have an anthology out called Girls Only that includes stories like Hidden Meaning and Maintenance.
Mist is a hardcore yuri magazine aimed at women, so you may find some things you like there as well. It is a little older, and the art reflects that.
Works is more plot than porn, but it's along the same general lines as the Mist stuff.
And if you want more titles like Shoujo Sect, I would recommend Yuri Hime Wildrose and the Yuri Hime cellphone line (Hanjuku Joshi, as someone mentioned, plus Gokujou Drops, Otome Senshi Lovely 5!, and Itoshi o Tome - Kimi ga Kokoro wa, among others).
You could also try I Fell in Love For the First Time.
