I am sure everyone has thought about it at some point, after seeing all we have I know everyone here must have an idea about what theirs would be like.
Basic criteria:
Name:
Sealed form: (this can basically be anything after Kyoraku and Ukitake's was shown)
Incantation: what would the phrase be to release its shikai form
Shikai power:
Manifestation: (what would it be in the physical world
Bankai power: (don't go saying that "If I can use bankai, then I don't need a incantation" just assume you learned bankai along the way )
I'll go first,
Name: Kyomeruame ("purifying rain")
Sealed form: wakizashi
Incantation: "Purge the lands, Kyomeruame!"
Shikai power: the wakizashi itself becomes water and the water itself forms into a rough image of katana and can change shape, density, and viscosity. More water can be controlled at the expense of more reiatsu. Water control range is limited to about 100 meters with accuracy and speed decreasing with range.
Manifestation: umm.....a huge box jelly fish? Yeah that would work out well 
Bankai: The wakizashi and any trace of it evaporates and instead the wielder gains complete control of water, shifting its state of matter from ice to water to steam or vapor with ease. Freezing at this state occurs without air pockets, causing the ice to be denser and stronger than steel (hell it sunk the Titanic). The range of control has not been fully tested but there is no penalty for distance. In close range, blood in the body can be manipulated and controlled as well. (There are limitless option in that by itself  ).

I would never put so much time into imaging my own zanpakuto  sheesh, screw the zanpakuto, I just want to work for Mayuri.

shikai: Becomes a double bladed ligthsabe like in Star Wars. I can also control the size and length of the blades
manifestion: Super hot naked chick
bankai: lightsaber blade breaks into tiny parts depending on how many enemies I have and each one peirces the heart of one of my enemies.

That quiz is only on existing zanpaktou, I mean make you own completely. You can't make a quiz out of that

manifestin: old cowboy guy
Bankai: A become a cannon that hv 100% accuracy and can shoot like machine gun and like shot gun. also can fight melee because got lightsaber.

Shikai: The katana itself become and invisible wind blade.
Manifestation: A Miko (Pristess in her kimono =P )
Bankai: The Katana becomes one with me and I'm able to control the wind (much like the main character in kaze no stigma)

Shikai: The katana's blade is made out of light, extremely, extremely thin and flexible, and can do the same thing as Byakuya's zanpaktou
Manifestation: A white wolf
Bankai: The katana transforms into a white wolf that's ten times larger than human size, made out of light and can do everything its shikai form can do but better, and I can ride it

shikai: the microphone doubles in size and will use sound as a weapon with limitless range.
manifestation: biggie smalls
bankai: Frank White himself comes out and handles the microphone with much talent and charisma.

Incantation: Pierce the heavens and strike true
Shikai power: Takes the form of a spear with electricty flowing through the spear head. Can be freely manipulated by wielder and launched at opponents. When lauched it will always strike the opponent it is aimed at unless stopped by wielder. Can only be carried by its true owner, no one else can even lift it. Can produce up to 7 lightning spears simultaneously that can also be freely manipulated. Limited lightning manipulation besides spears such as launching lightning bursts.
Manifestation: Odin, the god from Norse mythology, riding on his eight legged horse, Sleipnir
Bankai power: Kind of a senbon zakura ripoff with multiple forms. General power and appearance: Lightning manipulation through Gungnir and Black/Gold armor with helmet (kind of like Komomura).
1) spears of valhalla: summons 540 spears of lightning that can be manipulated freely by wielder.
2) walls of asgard: manipulate reiatsu to create a bunch of nearly impenetrable one sided walls that can act as a shield or a prison. By one sided I mean one side is impenetrable, but stuff can pass through the other side. Kinda like a force field.
3) Final Form: ragnarok: focus all reiatsu into gungnir and launching it at the opponent at very high speeds, destroying everything in it's path. Gungnir grows in size due to the amount of reiastu being released.

Full name: Hangyaku Daragon (The Rebel Dragon)
Sealed form:Katana slung on back
Incantation: Eliminate, Hangyaku!
Shikai: A kind of curved blade like renjis zabimaru,but without the spikes, extremely flexible like a whip and can split almost anything into halves
Manifestation: A dark figure made completely of shadow, nothing visible but red eyes and the silhouette of a dragon
Bankai: A huge, curved blood red blade, when swung it lets out giant jet black spirit force, with three different forms in which to transform:
1: Basic stage, normal curved blade
2:The blade moves up the actual sword and forms a spear
3:The blade splits in two forming a crescent moon on the tip of the sword

Sealed:A Handle, with a 1ft long blade.
Shikai: Dual Blades, One of the Blades are 1Ft long wile the other is more of 3,5 ft long, Attached to each other With a Thin Yet Extremely Solid Chain.Hells Leech; with the power to summon Spiritual Leeches. the amount of leeches Depends on my spiritual strength, that I use to summon them. The Leaches Attach them self to whomever they strike and Slowly Drains the Spiritual Power off the Victim. The leaches also adds a small amount of poison Which Makes the Victim unable to feel the Drain. Once the Leaches are Full, they die with a Burst/Explosion of the Spiritual Energy it Drained. Amount of what it can hold is also depending on me Spiritual Energy.
Manifestation: Creepy old, Rotten, bony, Japanese Type of Dragon.
Bankai: looks of a Scythe. just Deadlier Blacker and looking for Blood
Field Drain; Slowly Drain Energy from Any surrounding spiritual Beings and Adds it to my Strength.
Ghastly murmurs; Slowly With the Drained Power summon Vivid images of monsters/ghosts from the mind of the Victims and make them Paralyzed with fear.
