The Shingo and Kumiko series is exactly what you're looking for. The guy and girl have been together since he was 10 and she was 16. They're still together in the final chapter which is set 10 years later.

Dance in the Vampire Bund has a nice shota side story (fills up a whole volume, and they appear throughout the series) but the main focus isn't on them.
And their relationship becomes a tragedy in the later volumes.
But that's the closest to shota I've ever read.

Rising Impact, though romance is just a subplot.
The kid is 9, the woman is 21 and there is NO pedophilia of any kind.
Unfortunately said woman appears very little after the 2 first volumes. The manga ends with a time skip and guess what? Those two are married.
