This is false. None of them die, they simply leave the Earth with the Kaballa and leave the rest to the readers imagination.
For an anime you could check out Gungrave... It may even have a manga, i don't know for sure.
Don't forget Gantz... I'd spoiler tag it, but the entire premise of Gantz is about dying and being revived, so no reason really.

the perfect manga for that is
death note
its kinda scary and alot of people die, and the main characters dies in the end for his own selfish reasons, and by the same thing that he uses to kill other people.
its totally awesome. in the anime, it had so much blood [ kinda didnt like it, im not a blood person ]...some people might think its cool.
highly suggest it!

Only thing I could think of and didn't see listed was Bakune Young. Weird, weird manga.

yeah I read through this yesterday, was quite interesting imo, too bad the scans are till volume 4 only
I definitely recall reading all 9 volumes scanlated. I think they were on OM, so hard to say if you'd still be able to find them. They're probably around somewhere.

Eternal Sabbath fits, too.

Didn't see it mentioned, checked through the thread a few times (though skimmed, as I wanted to avoid spoiling a few of the manga).

Angel Sanctuary - the ending's kinda open, but what else could have happened?
Freesia - again, the ending's confusing, it's a case of who could survive that
Godchild (Count Cain's final arc)
Threads of Time
Bokurano - though the manga's premise is kinda this
