Anything by Yazawa Ai, really. Although she can be so realistic that it's gets boring sometimes...
Nana and Paradise Kiss are two that come to mind by her.
Also: Sunadokei and Kimi Wa Pet are pretty realisitic. =)

Kare Kano has a fairly realistic romance (it's relative in manga). The art isn't realistic tho.
