I'm looking for a manga where death follows the protagonist(s) like a shadow. Like through their actions or inactions people die. For instance, the protagonist fails to prevent someone's death, indirectly causes someone's death, or is forced to kill. The person(s) who die can be either guilty or innocent victim or perpetrator.

Death Note maybe? there's a lot of death, and the protagonist is causing them, on purpose.
its not an action manga, like what some of your's seem to be, its more pyschological.
also, maybe Chrono Crusade, the death isn't the main theme, but its still there. and its not right at the beginning. It kinda reminds me of Trinity Blood.
Still haven't completed it myself but till far seems good, and it has to do with both mystery and some killings.
Spiral is the name... if some1 has read it maybe can add more comments about it

Freesia should be right up your alley. Its a near-future story of a Japan where government assassins are hired by people to exact revenge on the murderer that killed their loved ones.
