I vaguely recall reading this school-life type manga where the uke was very unattractive, with very distinct, rat-like teeth. It's not Aitsu no Daihonmei, and it's not Mawaru Sekai no Kimi to Boku, although the premise is similar to both of them. I'm pretty sure that the seme moved away in elementary school and forgot about the uke, and was a more popular guy when he came back.

Other details that I'm not entirely sure about: the uke may have been a bit of a bully when they were young. There was something about him being so ugly they put a paper bag over his head?

It's such a distinctive story, but I can't find it and it's driving me crazy.
