Royal Koyanagi, Inu, Tony Taka, and Yuzuki N Dash may be what you're looking for.

Itosugi Masahiro's works usually have over-the-top stories, especially when it comes to girly boys, but the intercourse itself is actually rather tame. Though sometimes he may exaggerate the amount of body fluids, I still find it entertaining.

For actual art style itself, I can only think of Naruko Hanaharu in terms of realistic physiques and proportions. Though if you mean the shadowing and detailed backgrounds, it tends to vary drastically between his B&W and Color pages.

For backgrounds, I'd have to choose Kamirenjaku Sanpei (Ono Toshihiro), but his stuff is all weird as heck... So don't bother with him.

If you want my honest opinion for the mangaka with the most promising art style and stories, I'd have to go with Rokuroh Isako. He(?) draws well, and none of the sex have been too outrageous, as far as I can tell. Except for the beastiality and incest ones.... Okay, nevermind what I said then.

If you don't mind the loli genre, Sekiya Asami is getting quite popular nowadays. Despite the negative connotations associated with loli -- usually synonymous with rape -- the stories are actually very serene and adorable. One just needs an open mind. However, the art may not be on the level you are seeking.

Shinonome Taro also draws quite well, though I haven't seen anything from him since Swing Out Sisters... It's too bad.

If the genre or manner of publishing doesn't matter to you, then might I also suggest the doujin circle "ponz.info"

I agree with Sekiya Asami and Naruko Hanaharu suggestions from the previous post.

You can also try Tsukino Jogi (especially flat 38c - Loveberry Twins) or Inu (I liked Hatsu Inu) or Tomohiro Kouda (if you don't mind loli I suggest Petit Roid 3 since it's quite funny and it has a decent plot for the hentai standard) or Kisaragi Gunma (very gorgeous girls in all his works).

And if you don't mind something that it's not properly hentai I think you'll enjoy Yuria 100 Shiki.
