I always thought that it was just a fancy way of saying, "What do you wish for?"
If that makes any sense.
Like, the "xxx" is a variable or a fill-in-the-blank, and the Holic can mean addiction/desire/wish.
I'm also pretty sure that Clamp's aware of the "xxx" in the title possibly reffering to porn. xD For example: Maru and Moro's full names translate to "stripping" and "flashing."

the title of this manga "xxxholic" is sorta explained in the anime movie of xxxholic.
xxx means forbidden, restricted, and anything that seems weird or adult-like.
holic means a person who has a enormous desire to collect things.
so basically xxxholic is probably referring to yuuko's collection in the story.

I have wondered that for a while, and I don't know.
