I couldn't find anything with all of the criteria, but I found a few that are close:

A Stranger in My House - guy used to be in love with her mother, but he's not really a guardian and they are close in age.

Tsukikage Baby - girl is in love with the man who was in love with her mother, but he doesn't love her back and only sees her as a stepdaughter.

Yume no Moribito - guy is like a guardian/caretaker to girl

Hanatsukihime - guy is a immortal demon who raised girl (this is my least favorite of all of these)

Meteor Methuselah - guy is immortal and rather protective of heroine. He does have a long dead love that he feels guilty about.

edit: And I forgot about Mahoutsukai no Yome. It's not finished but it seems to be heading in the guardian + heroine direction.

And also a general name that is associated with heroine/guardian stories is "Daddy Long Legs" after the 1912 Jean Webster novel (which seems to be more popular in Asia for some reason). It could help with finding titles of interest.
