Probably not of much use but... the chibi's looked interesting to me too, so I looked back and the earliest time I could see that Essence of Purity had used that title page was the 4th chapter of Lovely Sick on 8/25/04 - I looked at manga they'd released just before and after, but nothing looked like that at all.
Might be worth checking more of their manga if you still want to know - otherwise they might have commissioned it especially.
