Manga: Naruto (I only read manga now because I found out Naruto was one of them)
Anime: Pokemon, maybe Yu-Gi-Oh! but it was probably my second.
I didn't even know what anime were, when I first watched them, so for a long time I thought my first anime was Shaman King and then Naruto... Because that was the order they were shown in on Jetix. Shaman King at 8:00 pm and Naruto at 8:30pm.
EDIT: Digimon might have been one of my first anime as well, now that I think about it.

The anime that got me into anime was Tokyo Mew Mew about 5 or so years back.
As for manga... I've read far too much to remember.
