When I watched to much anime during holidays or even on weekend, I can remember particular words only in Japanese. It happens quite often that I say e.g. "arigatou", "onegai" or "ohaiyo" and I don't even notice. My friends are used to it by now, but recently I talked japanese to one of my teachers and I wondered why he looked at me kinda irritated. After the lesson a friend of mine explained it to me... I really haven't noticed!

You definitely start comparing people in rl to characters you love. And don't forget carpel tunnel syndrome. evil thing, that is.
Every time some one is about to take my picture I make a V with my middle and pointer fingers. I'm not even conscious of it until I see the pictures developed. That and I tend to close my eyes when I smile.

I've been watching anime and reading manga for more then 10 years, and the only physiological effect I can see is that I'm starting to see not so well anymore. My vision was perfect, but now, if I need to read a text that is 30 meters away it'll start to become blurry. Well I also work in the IT, so I'm using a computer most of the time I'm not sleeping. And I read most of my manga on the comp screen, so...
There are plenty of side effects if you do not pay attention, especially nutrition wise (being an otaku doesn't leave you so much time for the rest...). I've started to pay attention to that a few years ago.

Words that are new to me I end up trying to pronounce as if it were romanji, so the way I talk creates communication problems sometimes.
I also have developed my own japanese-based onomatopoeia, like if it is raining I'll think something like "piru piru piru".
I have no idea how big breasts need to be to be considered big in real life, either, thanks to manga/anime females.
I suddenly needed glasses a few years ago.

I start to pair up the guys in my class together 😲 That is NOT normal! I think I need therapy. Trying to speak in Japanese or convert every day conversations with your friends to Japanese. Oh, wanting to establish your own doujin circle.

I want to make my own manga. Sometimes when I'm walking around I make random hand signs and shout out stuff like ameterasu and kage bunshin. I've tried using hadoken on my little sister. I nearly pulled a spinzaku at school (I only got 180 around) Whenever I see 2 guys talking to each other I pair them together. I get shining collection stuck in my head all the time. When people ask me about my religious beliefs I always say haruhi/lainism. Finally whenever someone talks to me and I dont feel like having an actual conversation I will say "To right the countless wrongs of our day. We shine this true light of redemption. That this place may become as paradise. What a wonderful world such would be"

Realistically: Bed sores? Sitting on your butt for several days straight without moving can cause the pressure and sheering force to give you bed sores? Anyone who doesn't know what they look like, google it, it isn't pretty.
Severe muscle atrophy? The lack of muscle use may cause you to become weak, fat, and feeble like a child?
Unrealistically: Although the radiation of a monitor is comparable to the radiation of a lightbulb, perhaps you can get super powers?
Maybe you'll suddenly find yourself in a fantasy world fighting evil villains and using secret moves to kick their butts, then winning the hearts of an entire harem of girls only to wake up and find out that you had comatose for a few days?
Smell really bad? I don't know, there are many things wrong with sitting in front of the computer (but that isn't going to stop me!)
