Hello! I'm looking for something along the lines of MPD Psycho and Leviathan, but with more horror themes (vampires, ghost stories, monsters). I am a fan of grotesque imagery, but I prefer it when served with a thought provoking plot.

Thanks, but I've actually read both of those! I've read what is probably the vast majority of the grotesque and horror psychological manga that I can find, and I was wondering if anything had slipped past me.

I'd also be willing to look into untranslated series if anyone knows of anything new or old that is awesome and twisted.

Since it wouldn't be on your list anyway, have you watched the Kara no Kyoukai films that are out already? The third one is pretty awesome.

Looking at your reading list so far (I'm not sure if that's all that's on it, but I'll give recs now anyway.

I don't see any of Higurashi no Naku Koro ni's chapters.

Other than that, you could try:
Hitsuji no Uta (highly recommended, although only mildly grotesque and more just horror/psychological.)
Nijigahara Holograph (see above, but the only real symbolic images are butterflies, despite there being murder scenes.)
Elfen Lied
Umineko no Naku Koro ni
Hoozuki no Shima (nothing grotesque yet)

There's probably more, but I can't think of any at the moment.

I can't say if you're already read them, but:

Kiseijuu
Shingetsutan Tsukihime not grotesque, but with vampires.
Rabbit Doubt
Kurozuka
Drifting Classroom I never read it 'cause it's not licensed in my country and I don't think scans will be revived so it's useless for me to start, but many people suggested it here and it seems really good. Maybe you can try it if it's licensed in your country.

Of course also Higurashi as already mentioned here. Same of them are not so grotesque or psychological, but I think they worth a try.

Thank you two for your recommendations! I'll look into all of those titles. I would like to read Higurashi but I don't know where to start since there is so much of it.

Higurashi actually has an order with its chapters, but you can read it in any order you like.

Officially, I believe that it's:

"Question arcs" (these introduce you to the story, so try to read these first.)
Onikakushi-hen
Watanagashi-hen
Tatarigoroshi-hen
Himatsubashi-hen

"Answer Arcs" (the first two answer most of the questions presented in the intial arcs, while the last two show the conclusion).
Meakashi-hen
Tsumihoroboshi-hen
Minagoroshi-hen (read this second to last).
Matsuribayashi-hen (read this last.)

All the other arcs are extra, you can read them whenever. Alternatively, you can just watch the anime, since that's completely subbed.
