Tokyo Crazy Paradise has a disabled character in it at one point...

And if yaoi's okay, there's Agnus Dei - Lamb of God.

Nodame Cantabile I think. I haven't actually read it but I heard the the female protagonist is somewhat mentally abnormal (not sure if that would count as disable...)?
Othello (shoujo) if you're into split personalities/ Schizophrenia...

Maybe Until Death Do US Part by TAKASHIGE Hiroshi/ SONG Ji-Hyoung or Gangsta. by Kosuke? Both are dark and gripping action series, the male lead in the former is blind, and one of the protagonists in the latter is deaf, but they aren't really bothered by their disability.

The one-shot Koe no Katachi by Ooima Yoshitoki portrays the grade school life of a deaf girl. It's about bullying and managed to capture both sides, the one of the victim and the one of the bully, so spot on that Japanese publishers refused to feature this award-winning story.

By the way, there's also this other thread, Main Character/ Love Interest has disability or a special ability. whose OP asks for romance with a disabled person. It might provide you with more ideas.
