I think the hentai tag is confined only to heterosexual couples. I've seen hentai that happens to include yaoi along with hetero (and sometimes yuri), but I've never seen a solely boyxboy yaoi with a hentai tag.
I have, however, seen yaoi that are pretty much hentai in every aspect except that the girl is a guy. The only one I can remember exactly is Moyashimon dj - Because You're So Cute. There was also a Reborn yaoi doujin I once saw that had a slight hentai-esque style to it. Doujins can be really hardcore - I recommend looking through them even if you're not familiar with the original series.

I guess the only REALLY hard yaoi would have to be the male-oriented shotacon...
